Police Department Forms

• OPRA Request for Public Records  Form MPPD-OPRA: To request Crash Reports, Incident Reports and other public records.

 

• Vacant Home Check   Form MPPD-VHC: To request security checks of your home while away or on vacation. Vacant Home Check forms must be turned into the police department by the requestor in person.

 

• Business Emergency Contact Registration   Form MPPD-BECR: For all businesses as they must provide a list of emergency contacts.

 

• Alarm Registration   Form MPPD-AR: Per local ordinance all burglar and fire alarms must be registered with the police department.

 

• Bad Check Questionnaire   Form MPPD-BCQ is required for all cases involving citizen complaints of bad checks under N.J.S.A. 2C:21-5.

 

Firearms Application Forms

 

• Consent for Mental Health Records Search   Form SP-66 needed for all firearm applicants.

 

• Application for Firearms Purchaser Identification Card   Form STS-33 required for all persons wishing to obtain a Firearms Purchaser Identification Card.

 

• Application for Duplicate Firearms Purchaser Identification Card   Form STS-3 required for application to replace lost or stolen Identification Card, to replace mutilated Identification Card, for change of address on Identification Card, for change of name on Identification Card or for change of sex on Identification Card.

 

• Application to Purchase a Handgun   Form STS-33 required for all persons wishing to purchase a handgun.

 

• Application for Permit to Carry Handgun   Form S.P.642 required for all carry permit requests.

 

• Certificate of Eligibility   Form S.P.634 is required for each transfer of a Shotgun, Rifle, including black powder or BB Rifle.

 

• Voluntary Form of Firearms Registration   Form S.P.650I is needed to voluntarily register a firearm.
